Healthcare/ Spirituality Survey

Dr. Jennifer Hamilton of the Family Medicine Department at the University of Medicine and Dentistry of New Jersey is conducting a research project, asking patients whether or not they'd like their doctors to discuss issues of religion and spirituality as they pertain to end-of-life care. There have been several such studies, all directed to the general population. This project is directed specifically to Neo-Pagans.
The survey itself is less than 25 questions long, and should take about 5-10 minutes to complete. It's completely anonymous -- the form doesn't even ask your name. (Some demographic information, like age, is requested.)
